Based on the responses Competent!NurseParctitioner!MyLittleSister has given to similar queries, I'd say that Competent!Doctor!John is likely to bundle Cousin Simon the hell off to A&E where there is a nice X-ray machine just waiting for its date with Cousin Simon's poor abused foot, and then some nice sterile equipment ready to deal with the fallout. Especially if Competent!Doctor!John is on his own vacation at the time.
